You will need to complete a registration form and a health questionnaire at your first appointment.
PLEASE BRING THE FOLLOWING TO YOUR APPOINTMENT:
Referral letter from a general practitioner or medical specialist
Correspondence letter from your physiotherapist or podiatrist, or any other parties involved in the care of your presenting problem
All relevant imaging and their reports (including x-rays, ultrasound, CT and MRI)
Medicare card
Private health insurance or TAC/WorkSafe claim details
The information from your registration form, health questionnaire and aforementioned documents will be recorded and used for the primary purpose of providing care for your presenting problem, including for administrative and billing purposes. It may also be disclosed to other health care professionals or involved parties with your consent.
Fees
Consultation fees are to be paid at the time of your appointment by cash, EFTPOS or credit card (Visa, MasterCard or Amex). For medical specialist appointments, you will require a valid referral from a general or specialist medical practitioner to claim a Medicare rebate for your consultation.
Out-of-pocket fees for your operation are to be paid prior to your surgery by cash, EFTPOS or credit card (Visa, MasterCard or Amex). An itemised quote for your proposed procedure, including any out-of-pocket co-payments, can be provided as requested. If you do not have private health insurance, you may choose to fund your own surgery with a payment plan as an option.
Urgent Referrals
Our practitioners will endeavour to see all new referrals within 7-10 working days. For urgent referrals, such as acute fractures, please inform our staff who will discuss with the relevant Orthopaedic Surgeon that same day.
TAC and WorkSafe Injuries
For patients who are awaiting approval from TAC or WorkSafe, consultation fees are to be paid at the time of your appointment and a receipt will be provided to you to claim back from the involved insurance company. Patients who have sustained an injury from a motor vehicle accident or at work, and who need operative management as a result of their injury, will require written approval from the involved insurance party prior to their surgery.
Uninsured Patients
For patients who do not have private health insurance and want to be managed through the public system, please ask your doctor to refer you to the outpatient clinic at the public hospital where the specialist works. Surgeons will not be able to book any patients onto their public operating waiting list until they have been reviewed in their outpatient clinic at the public hospital.
Cancellations
If you need to cancel your appointment, please inform us at least 48 hours prior to your scheduled consultation, so that we can offer your appointment time to another patient. Otherwise, a cancellation fee may apply.
